CDH Australia is run solely by volunteers and heavily relies on donations to help support CDH families and assist CDH research being conducted. All donations over $2.00 are tax deductible and funds help us to:
- support and invest in research and education programmes in relation to CDH
- provide training to our volunteers and produce support booklets so our volunteers can be better equipped to support our families
- provide telephone support and online support forums for families affected by CDH
- produce hospital information packs for expecting families
- facilitate an annual forum each year for families and medical professionals to share knowledge.

In order for CDH Australia to know this donation has been made in memory or honour of a child please send an email to donations@cdh.org.au to notify us that you have made a donation and who it is in memory or honour of.

Please print and complete our gift form and post together with your cheque or money order to: CDH Australia, PO Box 375, Morwell VIC 3840, Australia. A receipt will be mailed to you.
Making Donations In Memory Of An Angel Cherub
CDH Australia extends their deepest sympathy to you on the loss of your precious child. If you would like to talk to any of our families who have also lost their child to CDH please do not hesitate to contact us information@cdh.org.au

Tribute Certificates
All donations can be made in honour or memory of individual cherubs. If you would like us to send a tribute certificate in association with your donation, please email us and detail the tribute certificate type you would like sent and the contact details of person you would like the certificate to be mailed to. Tribute certificate types available include: standard, birthday, in memoriam and Christmas. Please note, the certificate is sent once payment is received and processed.
Hospital Information Pack Dedications
Hospital information packs are distributed to hospitals, doctors and clinics so they can give out these packs to families expecting a baby with CDH. Packs are presented in a glossy colour folder and include a cover letter (that contains extracts from our past newsletters), brochure on CDH Australia, our state contacts listing, membership form and donation form.
You can choose to allocate your donation to the production of hospital information packs and write a personalised message that will be printed on stickers and placed on the pack/s you donate. Please note the cost per pack is $5.
If you would like your donation to be allocated to hospital information packs, please email us your details and personalised message. We will endeavour to meet any special requests made by donors to supply packs with their messages to their preferred organisation, or to make arrangements for donors to personally distribute their donated packs.
